  • Flexible Funding Options: Let us know whether you’re using the California Self‑Determination Program (SDP) or choosing private‑pay services. Because we specialize in self‑directed care, you do not need a traditional regional center referral to begin working with us.

Step 2: The Consultation & Matching Plan

  • Meet Carmen: We schedule an introductory call or in‑person meeting to explore daily routines, current capabilities, and unique learning styles so we can understand how to best support you.

  • Align with Your Goals: We will review your Person‑Centered Plan (PCP) or Individual Program Plan (IPP) to ensure our skill‑building milestones align with your long‑term vision and support your overall independence journey.

Step 3: Onboarding & Scheduling

  • Seamless Onboarding: Complete our welcome packet to finalize your enrollment and ensure we have everything needed to begin services.

  • FMS Authorization: If you are using SDP, simply notify your Independent Facilitator or Service Coordinator that you are hiring Living Well Essentials. We will promptly complete the standard vendor packet and W‑9 with your Financial Management Services (FMS) agency (such as Premier, Mains’l, or GT Independence) to authorize billing.

  • Set the Schedule: We establish consistent, recurring days and times for your direct, 1‑on‑1 home‑based and community instruction.

Step 4: Active Skill Building & Milestones

  • Direct Instruction: We provide hands‑on, individualized teaching in the home and out in the local community, focusing on practical, real‑world skill development.

  • Progress Tracking: We monitor milestones closely—whether it’s confidently navigating a Placer County Transit route or safely managing a weekly grocery budget—and provide clear, actionable updates your family can use to manage your annual SDP spending plan with confidence.
