The Mission Behind Living Well Essentials

Welcome to Living Well Essentials! My name is Carmen Adriana Pendergraft.

My entire career has been driven by a simple, powerful belief: everyone deserves the opportunity to build a life defined by their own choices, capabilities, and independence. Over the past 15 years, I have worked deeply within professional training, coaching, and curriculum development. I’ve spent more than a decade creating structured educational programs, and if there is one thing I have mastered, it is how to break down complex everyday concepts into simple, manageable, and highly individualized steps.

As a direct Independent Living Skills (ILS) Instructor based right here in Lincoln, I don't just teach routines—I build relationships. I partner closely with individuals, their families, and their broader circle of support to set meaningful goals and celebrate the real-world breakthroughs that happen along the way.

I know firsthand what it takes to guide adults through the daily hurdles of independent life. Whether we are practicing how to balance a household budget, mastering kitchen safety, or building the confidence to independently ride Placer County Transit routes, I am right there in the community doing the hands-on work alongside my clients.

A Truly Person-Centered Approach

Independence isn't a one-size-fits-all formula. What looks like a success for one person might look completely different for another. Because of this, my coaching is entirely customized to align with each participant’s unique Person-Centered Plan (PCP) or Individual Program Plan (IPP).

Together, we focus heavily on three pillars of self-directed growth:

  • Daily Living Foundations: Building true comfort around nutrition, healthy meal preparation, personal care, and maintaining a safe, organized home environment.

  • Financial & Digital Safety: Learning practical money management, handling a grocery store budget, and learning how to safely use digital tools and online community resources.

  • Community Mobility & Connection: Boosting the social confidence and physical navigation skills needed to access public spaces, manage medical appointments, and feel at home in the Placer County community.

Safety and Compliance First

Your family’s peace of mind and safety are at the center of everything I do. To ensure the highest level of trust, I maintain fully up-to-date CPR and First Aid certifications, strictly adhere to all local licensing standards, and have cleared thorough Department of Justice (DOJ) background screenings.

Navigating state support options can feel overwhelming, but you don't have to figure it out alone. Because Living Well Essentials focuses directly on self-directed care, we are fully available to families utilizing the California Self-Determination Program (SDP) as well as Private-Pay clients.

You do not need a traditional regional center vendor referral to work with me. If you are using the SDP, we will simply handle the standard onboarding paperwork with your Financial Management Services (FMS) provider to get started.

I am here to serve as your dedicated, local partner. Let's work together to build a confident path forward.
